CAPP and PREP Toolkits for Supervisors

The CAPP and PREP Toolkits offer resources, background material, and tools to Health Educator Supervisors. Multi-year projects like CAPP and PREP can expect staff turnover. Turnover tends to greatly disrupt project functioning and progress, especially when there is no hard copy of the project's goals, objectives, procedures, guidelines, processes, and assessment tools available. To address this challenge, ACT for Youth developed these Toolkits both as a reference for current supervisors and an aid to new project staff. In 2017, one Toolkit binder per project was distributed at Provider Day. We recommend that Health Educator Supervisors keep their binders updated so that the toolkit can serve as the institutional memory of your project.

Here we provide:

  • PDF files of each content area (corresponding to the tabs in your binder)
  • Word files for each tool or resource that you may want to customize
